When you create a form in the Form Builder, you choose one of two form types: Registration or Evaluation. Each type is designed for a different audience and purpose. In this article, you’ll learn what each form type is used for, how they differ, and how to choose the right one.

What is a Registration form?

A Registration form is designed to be filled in by candidates. Use it when you need to collect information such as name, contact details, qualifications, documents, or any other details before an assessment or event. When a candidate opens a published Registration form, they see the form fields along with a submit button to send their responses.

What is an Evaluation form?

An Evaluation form is designed to be filled in by evaluators and interviewers. Use it when you need them to score candidates and record structured feedback during or after an interview. Unlike Registration forms, Evaluation forms don’t display a submit button to the respondent since it is there in the parent platform.

How to choose the right form type?

Ask yourself who will be filling in the form:
  • If it’s candidates submitting their own information, choose Registration.
  • If it’s evaluators or interviewers recording feedback on a candidate, choose Evaluation.
You select the form type when you first create a form. This choice can’t be changed later, so it’s worth deciding before you start building.
