# How to report an exam incident for a candidate

When an issue occurs during an exam session such as a candidate complaint, suspected cheating, or a rule violation, you can report an incident against the specific candidate using the three-dot menu on their row in the workflow tab. The incident is logged in the candidate's profile timeline and includes details such as severity, workflow step, and actions taken.

In this article, you will learn how to report an exam incident in Center dashboard.

### Before You Begin

Make sure you have:

* Logged in with the appropriate role credentials (Center Admin, Center Manager, Proctor Lead, or Proctor) to reflect that other roles can also perform candidate actions.
* Opened a center and navigated to a **workflow tab.**
* A candidate in **Checked-in** status (incident reporting is only available for checked-in candidates).

### Step-by-Step Guide

**1. Open the actions menu**

From the workflow tab, locate the candidate you want to report an incident for. Click the **three-dot menu (⋯)** on their row.

**2. Click Report incident**

Select **Report incident** from the menu. This opens the incident report form. If the option appears greyed out, the candidate's current status doesn't allow incident reporting, only candidates with a **Checked-in** status can have incidents reported against them.

**3. Fill in the incident details**

Complete the following fields:

* **Nature of Incident** - select the type of incident (e.g., "Candidate Complaint", suspected cheating, unauthorized devices, rule violation).
* **Severity** - choose the severity level: **Low**, **Medium**, or **High**.
* **Select Sub Test** - specify the workflow step the incident relates to. While the label in the side panel says "Sub Test", this refers to the specific workflow step that was active when the incident occurred.
* **What Happened** - describe what occurred. Include the time of the incident for reference.
* **Action Taken** - document what action was taken in response (e.g., warning issued, exam paused, materials confiscated).
* **Reason** - provide any additional context or reasoning.

**4. Submit the incident**

Click **Submit** to log the incident. The incident is recorded in the candidate's profile timeline as a **red-dot entry** with the timestamp of when it was reported.

**5. View the incident report**

To review a reported incident, open the candidate's profile by clicking their name. In the activity timeline, click on the **red-dot incident entry** (e.g., "Candidate Complaint") to open the full **Incident Report** showing all the details you entered.

**6. Return to the profile timeline**

To go back from the incident report view to the main profile timeline, click the **← back arrow** at the top of the report.

  **Additional Tips**

  * Report incidents as soon as they occur, include the exact time in the **What Happened** field for accurate records.
  * The **Severity** level helps prioritize incidents during review. Use **Low** for minor issues, **Medium** for moderate concerns, and **High** for serious violations.
  * Incidents are recorded in the candidate's profile timeline and remain visible for future reference.
